To make the most out of your child's initial evaluation, follow these helpful
tips:
Arrive early - Please arrive 10-15 minutes before your first scheduled appointment.
Fill out the paperwork before your visit - In order to expedite the check-in process,
please print, complete, and bring the New Patient Information Form with
you to the appointment. If you have any questions, contact one of our centers.
**Please note: Each center may have additional forms for you to sign upon your arrival.
Gather your information - As an active partner in your child's success, you play a
very important role. Bring any additional information such as medical history or school records that will
help our therapists treat your child.
We accept all major insurances as well as Washington State Medicare and Medicaid.
